We need to have a copy of your state's tax and use permit and a copy of a completed Form 01-339 on file. We will be unable to activate your account if you send only one of these forms to us, and it will cause a delay in activating your account or cause us to delete your wholesale account. We will need to have both of these forms on file.

  Please fill out with your states information in the blanks. When you have completed this form please fax, email or snail mail Form 01-339 and a copy of your state's tax permit.

Tax numbers come in differnt forms or called differnent names example of these are as follows:  In the United Kingdom, England they us the word VAT number or in Australia they use Australian Business Number(ABN) these are just a few examples. when filling out these forms please use your Country's information in the blanks on the Form 01-339.
